Cayuga Counseling Services, Inc. is seeking an Care Manager/Adult Case Manager to join the Agency. this position is Full Time/35 hours per week and located at the Main Office in Auburn, NY . The Care Manager portion of this position serves as a the primary point of contact with enrolled child and their family to provide support to assure that the care coordination network of providers deliver comprehensive, integrated physical and behavioral health care. The Adult Case Manager portion of the position is to provide services to adult clients that restore, rehabilitate and support the individual client as an active and productive member of his or her family and community. The Case Manager will assist the individual in acquiring, retaining and improving skills such as communication, self help, domestic, socialization, self-care in order to successfully reside in a home and community based setting.


  • Outreach and engagement to formally enroll referred individuals into the care management program.
  • Coordinates and facilitates ongoing Care Plan team meetings that include the child, family, natural supports and any/all network providers assisting in the delivery of the Care Plan.
  • Ensures that all of the initial linkages are established and maintained and advocates for additional services and linkages as appropriate.
  • Receive HCBS/CORE referral from licensed practitioner or authorized referral source and initiate contact with identified client/family.
  • Provide rehabilitative services identified on the treatment plan which may include social and interpersonal skills, daily living, skills and community integration.
  • Services are provided in a variety of community locations where the child/youth/adult lives, works, attends school, engages in services and/or socializes.


  • A Bachelors of Arts or Science Degree in psychology, sociology, human services or related field with two years of relevant experience, or
  • A Licensed Registered Nurse with two years of relevant experience, or
  • A Master of Arts or Science Degree with one year of relevant experience. (Relevant experience includes proficiency in the human services or medical field in the area of mental health, substance abuse, public health, early childhood or established proficiency on an individual applicant basis).
  • Must possess the ability to organize and prioritize duties to meet job demands, make independent decisions, successfully engage children and their families.
  • Basic Computer Skills
  • Travel is required. Mush have a valid NYS Driver's License
  • Knowledge of the child serving delivery systems preferred.
